CHC Lifestyle Medicine Program
Lifestyle medicine is an evidence-based approach to preventing, treating, and even reversing chronic conditions, including cardiovascular disease, hypertension, Type 2 diabetes, and chronic obstructive pulmonary disease (COPD). Using education and group support, it focuses on replacing unhealthy behaviors with positive ones in the six pillars of lifestyle medicine:
- Eating a whole-foods, high fiber, and plant-forward diet
- Increasing physical activity
- Reducing stress
- Avoiding risky substances
- Making positive social connections
- Getting restorative sleep
Sixty percent of adults in the United States have a chronic disease, with 40% having two or more. Unhealthy lifestyle behaviors are the root cause of many of these diseases. In fact, diet quality is the single leading cause of premature death in the United States, causing roughly 500,000 deaths each year. The good news is that by changing the lifestyle habits that caused the diseases, you can treat or reverse the disease.
CHC’s Lifestyle Medicine Program empowers patients to make lasting and positive lifestyle changes that promote optimal health and vitality. Using a holistic, person-centered approach to care, our lifestyle medicine team will work with you to understand your personal health goals, identify the resources and activities that will help you achieve those goals, and provide the motivation and support you need make lasting health changes.
