Recognized for Quality

Community Health Center is a Patient-Centered Medical Home (PCMH), recognized by the National Committee for Quality Assurance (NCQA), which is a private, non-profit organization dedicated to improving health care quality.

As a Patient-Centered Medical Home, the CHC combines mission, teamwork and technology to provide you quality care and improve your health.

By being recognized as a PCMH, you benefit from:

  • Access to a consistent primary care provider and team of health professionals
  • Care that is coordinated and integrated
  • Being actively involved in your care
  • Our focus on quality care, patient satisfaction and safety
  • Access to care when you need it
  • Care that reflects our patients’ culture and beliefs — and our assistance in helping you overcome any obstacles to care
  • Help connecting you to services that are not provided within our medical home

The medical providers at the CHC are physicians, nurse practitioners and physician assistants. Nurse practitioners and physician assistants are healthcare clinicians who work under the direction of a physician. They perform exams, diagnose and treat acute problems, order diagnostic tests, and prescribe medications.

Our providers are all part of the care teams, which also include nurses, medical assistants, counselors, and registration and referral specialists. They work closely with pharmacy and dental staff to fully coordinate patient care.
