Community Health Center functions like any doctor’s office, offering services such as office visits, checkups, screenings and management for chronic conditions. But we also offer so much more — such as dental, vision and behavioral health services, along with an onsite pharmacy.
Our focus is on providing the very best healthcare, as well providing programs and education that help improve the overall health status of everyone living in our communities.
We are a non-profit provider of primary and preventive health services that cares for all patients, regardless of ability to pay or insurance status. If you don’t have insurance, our insurance enrollment specialist can help you determine if you qualify for any public health benefits programs and can help you apply. At Community Health Center, our care is centered on you — and everyone in our community.

The Community Health Center is a Patient-Centered Medical Home (PCMH), recognized by the National Committee for Quality Assurance (NCQA), which is a private, non-profit organization dedicated to improving health care quality.
As a PCMH, patients visiting the Health Center can benefit from:

The medical providers at the Health Center are physicians, nurse practitioners and physician assistants. Nurse practitioners and physician assistants are healthcare clinicians who work under the direction of a physician. They perform exams, diagnose and treat acute problems, order diagnostic tests, and prescribe medications.
Our providers are all part of the care teams, which also include nurses, medical assistants, counselors, and registration and referral specialists. They work closely with pharmacy and dental staff to fully coordinate patient care.
We accept new patients with all types of insurance — including MassHealth, Commonwealth Care, Medicare, new federal plans and private insurance — or patients with no insurance at all. If you need help finding insurance, call us at 508-477-7090.
