Community Health Center’s location in Centerville is a convenient office for mid- and lower-Cape residents, at the intersection of West Main Street and Route 28. We offer primary care and a walk-in clinic that is open to all, whether you are a CHC patient or not.

In 1998, Community Health Center began providing care to the uninsured and underserved residents as the Falmouth Free Clinic. Falmouth patients are still the largest demographic cared for at Community Health Center, and one in four Falmouth residents is a CHC patient. Our Falmouth office is located in Homeport, offering primary care, dental, and behavioral health services.

First opened in 2022, Falmouth Pharmacy is located directly next to our Falmouth office in Homeport. This is a full Retail Pharmacy and is open to both the public and CHC patients, with the ability to accept prescriptions from any provider’s office.

Community Health Center’s Mashpee office is conveniently located across the street from Mashpee Commons. The 32,000 square foot facility offers a full array of health services and services are offered on evenings and Saturdays.

Community Health Center’s newest location, our North Falmouth office is located only a few minutes from the intersection of route 28A and route 151. CHC is located directly on your right when entering through the front doorway. This office provides gynecological care, primary care, and lifestyle medicine.

Community Health Center’s location in Sandwich is located just 2.5 miles from the Sagamore bridge with easy access for off-Cape and upper-Cape patients and residents, located within the Sextant Hill Complex. This location offers primary care, behavioral health services, and substance use disorder services.
