Our team of skilled and experienced providers at Community Health Center work together to provide you with the very best health care — from medical and behavioral health to dental and vision needs.

Care is provided by physicians who are trained in family medicine and internal medicine, along with nurse practitioners or physician assistants, all working together to ensure you and your family receives comprehensive care in a setting that is comfortable, convenient and efficient.

Nurse practitioners and physician assistants are advanced healthcare providers who work in collaboration with our physicians to perform physical exams, diagnose and treat acute problems, order diagnostic tests and prescribe medications.

Our behavioral health services are provided by expertly-trained psychiatrists, counselors and social workers who develop treatment plans and follow-up care for patients.

Experienced dentists, optometrists and pharmacists complete our clinical team.

All of our providers are supported by nurses, medical assistants, hygienists, enrollment, registration and clerical staff. At Community Health Center, our entire clinical team brings their expertise, experience and compassion to care for all of our patients. Our care is centered on you at CHC — and everyone in our community.

Community Health Center has made the difficult decision to place a temporary hold on new patient applications. We, like many other primary care providers in the Commonwealth and the Nation, have experienced an overwhelming demand for our services and have faced significant workforce challenges following the COVID-19 pandemic. Over the last several years, CHC has seen double-digit growth in patients requesting our services. Currently, over 26,000 members of our community have chosen Community Health Center as their health care home. Despite the hire of over 5 new Primary Care Providers in the last year as well as other support staff, we have found it difficult to maintain access for our existing patients when they (you) need us.
