Community Health Center of Cape Cod, Inc. (“CHC”) is soliciting business and cost proposals from qualified firms to provide pre-construction and construction management through “Cost Plus A Fee” contract services for the construction of their proposed new health center to be located in Sandwich, Massachusetts (“CHC Sandwich”) in accordance with the requirements described herein. CHC Sandwich will be a 19,695 square foot outpatient facility (inclusive of 6564 square foot storage, mechanical and electrical basement) located at 85 Route 6A.

Proposals must be received not later than 3:00 pm, Friday April 4, 2025.

ADDENDUM #1

This addendum makes the following changes to the Request for Proposal issued on March 14, 2025. All other requirements outlined within the RFP Document and Drawings remain unchanged.
Proposals must be received not later than 3:00 pm, Friday April 11, 2025. One original and five copies shall be mailed or hand delivered to:
