Save Time Before Your Next Appointment with Pre-Check

At Community Health Center, we’re committed to making your health care experience as convenient and efficient as possible. That’s why we’re proud to offer Pre-Check, a simple way to complete important check-in tasks before your appointment.

What is Pre-Check?

Pre-Check allows patients to complete check-in tasks in advance from anywhere.

This service is available to patients who have a MyChart account, an email address on file, or a mobile phone number with Community Health Center.

Patients with one of these contact methods listed at CHC will receive a notification up to three days before their scheduled appointment, inviting them to begin the check-in process.

By completing Pre-Check before your visit, you can help reduce time spent checking in at the health center and allow your care team to better prepare for your appointment.

How It Works:

Step 1: Receive Your Invitation

Up to three days before your appointment, you’ll receive a notification through MyChart, email, or text message with a secure link to begin the Pre-Check process.

Step 2: Complete Your Check-In Tasks

During Pre-Check, you’ll be asked to review and update important information, including:

✔ Confirm your address and contact information
✔ Review medications and allergies

✔ Answer screening questions

✔ Update important health information

✔ Insurance Information (if applicable)

Step 3: Submit Your Information

Once you’ve completed the Pre-Check process, your information will be securely shared with your care team so they can prepare for your visit.

Step 4: Arrive for Your Appointment

When you arrive at the Community Health Center, you’ll still need to check in at the front desk so we know you are present for your appointment.

If staff ask you to complete paperwork when you arrive, please let them know you already completed Pre-Check so we can avoid asking you to answer the same questions again.

What Will the Text Message Look Like?

Patients who are eligible for Pre-Check may receive a text message like the example below with a secure link to complete their check-in tasks before their appointment. Simply click the link and follow the prompts to complete Pre-Check.

How Will I Know My Pre-Check Is Complete?

Once you’ve completed Pre-Check, you’ll see a confirmation screen like the one below. This confirms that your information has been successfully submitted and is on file for your upcoming visit. You’ll also be able to review important appointment details, including the date, time, provider, and location of your scheduled appointment.
