Duties include, but are not limited to the following:
Medically treat adult patients in an outpatient integrated model of care setting. Provide direct care to assigned patients on a daily basis, utilizing EPIC electronic health record and consultative services with specialists, including Behavioral Health Services, with providers here in the health center. Type of Medical Care: Internal Medicine.
Provide direct patient care services within area of expertise. Assess, plan, document, and evaluate patient needs and treatments. Ensure appropriate follow–up. Maintain a culturally and linguistically sensitive practice, using interpreters and other assistance as appropriate. Design a plan of care based on assessment of physical, emotional, and social needs in collaboration with the patient and, if appropriate, the family. Refer patients, as appropriate, to specialists and other practitioners and facilities. Review patient charts and ensure appropriate treatment plans are developed and followed. Work collaboratively with other caregivers regarding patient education and rehabilitation including promotion of mental and physical health. Maintain collaborative relationship with other physicians, administration, and clinical staff. Furnish professional guidance and assistance to colleagues and other staff. Utilize skills and expertise of other disciplines to ensure comprehensive care for all patients. Visit hospitalized patients as necessary; serves on rotating on–call schedules. Cover Urgent Care as needed.
Experience and Education include, but are not limited to:
• Doctor of Medicine or foreign equivalent
• Completion of Internal Medicine Residency
• Massachusetts Medical License
