Community Health Center is an independent Federally Qualified Community Health Center (FQHC) and a recognized 501(c)3 nonprofit organization. Your gift today will help us continue providing access to high-quality, affordable health care to those who need it most. Community Health Center now proudly serves more than 25,000 individuals on Cape Cod and beyond. We place a special emphasis on the underserved and underinsured in our area, and more than 70% of our patients are at 200% of poverty level or below.

Support from the community plays a crucial role in helping CHC achieve its mission: to improve the health of our community through the provision of equitable, patient-centered, high quality, compassionate health care to all, regardless of ability to pay. So much of what Community Health Center does, such as outreach, patient education and support, and addressing barriers to health that patients face, such as housing insecurity, food insecurity, transportation challenges, employment challenges, language barriers and access to affordable health care is not funded by traditional reimbursement mechanisms, making philanthropy all the more essential to achieving our mission.
