COVID-19 Vaccination Clinic – Moderna Bivalent Vaccine
BY SCHEDULING A VACCINE APPOINTMENT, I CONFIRM THAT:
- The patient being scheduled is at least 18 years old.
- I will provide my health insurance upon scheduling. My insurance will be billed at no cost to me. I do not need to be insured to receive the vaccine.
- I have not been exposed to someone confirmed to have COVID-19 within the prior 14 days.
- I understand I may wish to have a discussion with my healthcare provider(s) before vaccination if I have a condition or take medicines that weaken my immune system.
- I have read and agree to Community Health Center’s Notice of Privacy Practices and Consent to Treat
- I consent to opt-in to receive text message reminders to my mobile device regarding my vaccination appointment date and time.

You will receive a confirmation email upon scheduling, and the vaccine clinic will take place at the Pharmacy of our Bourne office at 123 Waterhouse Road.
